If you wish to apply for your child/children to be included in the 2020 Dress a Child, you can download the form below. All pages must be completely filled out.
If you want to e-mail them to us you can send the application, child's information form, copies of their social security cards or birth certificates and copies of your proof of income to uwba@bresnan.net.
If you want to deliver them to us and have us make copies, please call 782-1255 to make arrangements as the bank doors are closed due to Covid 19. Someone will be in the office from 9:00 - 1:00 everyday.
